¿Cuándo es tu cumpleaños? - When is your birthday?
Mi cumpleaños es _____. - My birthday is ____.
¿Cuánto años tienes? - How old are you?
Tengo ____ años. - I am ____ years old.

When you say the date, it's always:
"the day of the month"

May 4th - the fourth of May - el cuatro de mayo
December 29th - the 29th of December - el veinte y nueve de diciembre
June 1st - the 1st of June - el primero de junio

The 1st of each month is the only time you can't use just the number. And when you write the actual date, you can just put "el 4 de mayo" or "4/5"

¿Cuál es la fecha de hoy? - What is the date today?
Hoy es ___. - Today is ____.
Mañana es ____. - Tomorrow is ____.
¿Qué día es hoy? - What day is today?

¿Cuál es tu número de teléfono? - What is your telephone number?
